Street to Home
After Hours Nurses and a Clinical Community Health Nurse work alongside our Street to Home team to provide rough sleepers integrated outreach services across Brisbane, including nursing, care coordination, and case management.

Clinical Nurse Partnerships DV (CNDPV)
Our Domestic Violence Project stations a dedicated Clinical Nurse at the Inclusive Health and Wellness Hub to provide women who have experienced domestic violence with direct nursing care, immediate health assessment, education, care coordination and navigation through the health system.
Funded by St Vincent's Australia.
Brisbane Common Ground
Nurses employed by Mater Health provide seven-day-a-week nursing care as part of the Brisbane Common Ground team.
Funded by St Vincent’s Private Hospital Brisbane, Mater Health and Mater Foundation.
Home for Good
Under the Home for Good banner, nursing care is provided in parks, public spaces, boarding houses, people’s homes and in the community to address acute and chronic health needs.
